i was wondering what they look like and what information they contain?

The hospital discharge papers will include the patient's name, birth date, medical record number, the doctor's name, date of discharge, orders and instructions for further care at home as well as when the next visit to the doctor is if that is already scheduled. If it's not they may say on the sheet to call doctor's office to schedule a follow-up visit for after the patient comes out.
