How can I go about getting a psychiatrist's notes about me corrected?

I was going through the notes of a psychiatrist that had been seeing me and noticed a couple of errors regarding my past treatment history. Is there a correct way to get these errors fixed besides actually talking to the psychiatrist himself?

    If you can't talk to the psychiatrist himself, try the administrator.

    confront him or her politley and point out the errors

    and tell him or her the correct events and facts and request that they be changed

    or if you do not have the confrontation skills then alert through email, telephone or tell his or her assistant or colleague if necessary

    there is not specfiic protocoll for this event
