I did some research. There was a guy named Bob Timberlake that played as a QB/kicker for Michigan that was drafted by the Giants in 1965 and was a backup QB/kicker for one season. He made his first FG then missed 14 in a row and ended the season 1-for-15 in FG attempts. He was cut by the Giants and that ended up being his only season in the NFL.

The good: San Francisco, in basically all phases. Kaepernick was pretty much unstoppable and that defense is still great. Atlanta's running game. It's been hit or miss all season, but when they needed it, they had it. That and Matt Ryan was clutch on that final drive. Tom Brady and the New England defense. Brady was able to engineer long drives against Houston and Houston was unable to generate anything consistently on offense to keep up. Baltimore's big play offense and stout defense. Really kept Manning and the Broncos offense in check most of the game and the offense kept hitting the big play every time they needed it. The bad: Baltimore's special teams. Allowed 2 kick returns for TD's, and Holliday was barely touched each time. Denver pass defense and Manning. Denver, who hadn't given up but something like 3 pass plays over 20 yards all season, gave them up routinely and once again Manning's turnovers and their inability to sustain drives killed them. The offense only managed 21 points and 3 turnovers despite winning the time of possession battle. Packers defense in stopping the read option. Kaepernick breaks the NFL record for rushing yards by a QB and that defense looked cluess on how to defend that play all night.

Scott Sisson comes to mind.
He played for the Vikings and Patriots in the 1990's and had the nickname of "Missin' Sisson".
Though he did connect on 36 of his 55 field goal attempts.

Mike Vanderjagt was probably the worst playoff kicker I have ever seen and I think he held the Colts back while he was there.

He was a #1 draft pick bust out of Texas.
