It's all a personal choice, which depends on the level of hatred you still hold and the amount of trust you want to give.
Forgive and forget, to me, is not possible. You can forgive, but don't ever forget - because it's still a lesson that you must be wary of such a person. Forgiving the person is also something that you need to do for yourself; in order to find peace and carry on with your life. Unless the person had done an unforgivable sin, I would say the hand of friendship should be offered.
However, with caution. If you find that you can't get along with the person, even after trying, then so be it. You're not obliged to be friends with everyone.
As for "exes" - if you parted on good terms and both do not have any lingering feelings; why not? But if you think you're going to be caught up in the past and think of all the "what ifs" and "if only" - which also stops you from having a meaningful relationship with anyone; then no. A clean cut is best.
