i have to fill application form for passport, and i have no surname.There are three rows of boxes to fill,?
continue..uppermost row is for surname, Should i leave all boxes of this row blank ? If done so, will it not be a violation of instruction of not leaving any box blank ? OR should i fill my name in the uppermost row........ Please reply those who have personal experience of filling their passport form.

A. If you have only one name, a "first name", please enter your name in the column "Surnames" and write "FNU" [first name unknown] in the columns "First & Middle Names"..

I would say go to the actual passport office when filing your application ( you'll get your passport faster as well) there should be someone who looks over your papers and makes sure you have filled everything out and signed where you are supposed to. Ask that person what to do about that section.

You have to have a birth certificate or a certificate of nationalization to get a passport. Both these certificates will list your surname, and in the super unlikely event that your birth certificate does not, you can apply for a "delayed" birth certificate in the county and state in which you were born, and to do this you have to get witness statements from people who knew you when you were young. Start the process with the County Court Clerk. I had to go through the "delayed" process to get my birth certificate. The County Court Clerk knows all the details. In my case, I had no birth certificate at all.

I applied for a passport last year and I don't have a middle name. I left that blank and they accepted it.

even if u did a small mistake also the application form will be rejected so better to contact brokers near passport office....

Go to any RPO and lot of agents are available to get filled up with some token money

hey nothing lyk dat ...all are fake...listen if u dont have ur nick name in your other documents also lyk u will deposit ur 10th certificate ur ration card..and ol ...only then it will b accepted bt if u have in documents bt u r not filling in forms then it will not work..
