There are no True Democracies, where the people directly and collectively vote on governance. The closest thing we have are the Democratic Republics where the people elect representatives to advocate their will in legislative bodies.
A True democracy has no checks or balances - Majority rules, so the Majority can decide to disenfranchise the minority. It's mob rule with a process.
A true democracy is non-deliberative - which means that there is no opportunity for everyone to speak on an issue, and those that can speak are more likely to be powerful interest that do not have the interests of the people at heart. Look at many US state referendums and it is easy to see how the process has been hijacked by special interests.

I think Churchill sums up the search for the ideal Democracy : "No one pretends that democracy is perfect or all-wise. Indeed, it has been said that democracy is the worst form of government except all those other forms that have been tried from time to time"
I think the Swiss model through constant public referendums ensuring the government is of the people, by the people, for the people is the most successful of any democracy.
