Skin problems!! Please help!!!?

Ok so lately my face has broken out horribly and I don't know why. I have been using Clean & Clear detoxifying facial scrub in the morning followed by Clean & Clear astringent (sensitive formula) and then Clean & Clear Dual Action Moisturizer. I do this regiment twice a day except at night I usually use either the foaming facial cleanser or the deep action cream cleanser. I wear makeup too (which could be contributing to the problem) but I use Physicians Formula. So basically what I am asking is how do I clear up my skin or what am I doing wrong and I need to change?
