St. Bernadette describes Mary as carrying a white pearl Rosary with a gold crucifix in her apparition at Lourdes. When Bernadette prayed the Rosary, Mary let the "Hail Mary" beads glide through her fingers and only joined in on the "Our Father."
Mary also stressed the importance of praying the Rosary during her appearance at Fatima, but I don't remember reading anything about her actually carrying a Rosary.

I believe it was Our Lady of Fatima; but it could also be Our Lady of Lourdes. Statues representing these apparitions often show the Holy Mother with a rosary.
