why would a male say that its painful to fondle/get involved with their scrotum sac when i was under the impression this can increase pleasure...the man does have diabetes..never came across this problem/dilemma before. serious answers only please

If he's diabetic, he may already have a bit of neuropathy (a slow destructive process of the nerves) which initially makes the extremities which are effective hypersensitive.
And yes - it can affect scrotums and penises as well as toes, feet, fingers, arms and legs.

I've gotta say, i love it when a girl fondles my junk. That's including my sac. Hell, it might be cuz they have such a hard on, they have alot of built up pressure.

it can be very sensitive to pressure. you have to make sure that you're being gentle and not too rough. other than that, he might just not be into it. everybody likes different things.
