Yes, the Vocaloids are human (or at least human in form), so they have ears. The headphones are there for aesthetic purposes.

Basically, they don't.
Think about it, their human like robots. Like persocoms. They wouldn't put headphones over ears :\. But honestly I don't know, it's my opinion.
