How would I write someone's initials with a "Jr." in their name?

I'm trying to personalize a gift for someone who has a "Jr." - do I include this in the initials or not? If yes, how?

    You can by adding Jr after the initials. However, most people just leave out the Jr. when it comes to monograms for gifts.

    First, M, Last Name then Jr.

    John H. Doe Jr.

    My inclination is to leave it off, since, unless his father outlives him, he won't always be "Jr." In addition, it will be easier to take the present back to have "Jr." added if he really wants it than to have it removed if he doesn't.
