How do I file quarterly taxes without a 1099 in Ohio?

I am a nurse that works for a company that I am an independent contract for so I use the 1099 for taxes. I have to pay quarterly taxes but I am unsure what forms to fill out, how much I should pay, etc. Is there some type of estimator somewhere? I live in the state of Ohio. ANY information would be good, I am new to this! Thanks.

    The federal form is Form 1040-ES. The instructions tell you how to estimate the amount. Be sure to include the "self-employment tax" as well as the regular "income tax".

    that is 1040ES and you can find the instructions and forms at www.irs.gov

    you pay April 15, June 15, Sept. 15, and Jan 15 of next year what you anticipate your SchC 'net' will amount to for SE taxes(Sch SE) approx 15.3% of your Sch C 'net'

    and however this will impact your total gross income
