Look in Part 61 for the address, you can also call the local FSDO and explain the certificate number issue (lots of us don't remember our new numbers), they can find it under your SSN.

You will need to renew for medical. All you need is a 3rd class to operate an aircrft privately. Next flnd a local flight school and explain you need a Flight Review, but its been a while since you flown and you would like to have some refresher training so you can fly to PTS standards. I would say about 5-10 hrs and you should be good and be signed off for a flight review and be good to go. A certifacte does not expire unless surrendered, revoked, or obsolete. You can only excersise the privalages if you meet the recency of the experience requiremnts of Part 61. (takeoff/landing/flight review or approved wings program, or obtaining a new rating or certifacte)

Make out a check for $2 payable to the FAA. Write out a written statement with your name, date and place of birth, social security number, and an explanation of what certificate you need replaced and why you need a replacement certificate. Mail it to:
Federal Aviation Administration
Airmen Certification Branch, AFS-760
P.O. Box 25082
Oklahoma City, OK 73125-0082
Note that you only get to do this once, so don't lose the replacement. The new certificate will be valid from its date of issue, so it won't help if you need to prove you were licensed in the past.

The absolutely free reverse email lookup sites generally provide false information, they just want your email to send you spam. To get real information, money will have to be paid.
Also, stay away from shady reverse email lookup sites, most likely you won't get any information after you make the payment. Not to mention you won't get a report and you won't get an answer if you try to call for a refund. Stick with a reputable reverse phone lookup site like http://www.emailtracer.org/ that has been around since 1997.

I think there is a procedure to request a new license be issued that does not contain your SSN. I think they will do this change for you for free. Otherwise there is a fee.

Simple. Go to your local FSDO with photo I.D. and a logbook. http://www.faa.gov/about/office_org/field_offices/...
You might be able to do it online or on the phone with yout SSN:
