Its an Anaglyphic 3-D image, made by a camera with a cyan and red filter built in, the only commercial example that springs to mind is the long since discontinued Vivitar Series 1 70-210 Q-dos lens.
You may be lucky enough to pick up a used example on ebay or similar, though it was never made in Canon EOS mount or Minolta M/AF mount as far as I am aware.
You can mimic this effect by combining two images and there are various tutorials online, this is a good one:

Image was taken with a Canon 450D DSLR and the image was played wit in Photoshop CS2 (all this is in the EXIF data).
There are probably many ways to do this in Photoshop, but I would create two further copy layers from the original image, change the colour of them and off set them slightly, then merge.

I have no idea what the effect is called, but no camera to my knowledge will do that. It was probably altered in Photoshop, with a higher saturation level and a bit of a cooling effect.

Looks like a 3D effect. They use it in magazines and other printed medium, where they usually give away 3D glasses with the publications in order for you to view those pictures properly. I bought such a magazine a couple of months ago where the glasses were included.
