Listen to the horse's heart and lungs, watch them for any signs of lameness (while someone leads the horse), perform flexion tests to check for any resistance, possibly check the horse's teeth; basically it's an in-depth check-up. Some buyers will also have x-rays taken of the horse's feet/legs.

Lameness exam, flexion test, listen to the heart/lungs/etc, look at their...'parts' (especially for mares), look at the eyes, ears and mouth.
Definitely need to get one!
