I need to know what I can do to get things I need (17 year old getting kicked out)?

Hello, I am looking to get help in whatever way I can however I can. I am 17 and barely able to live at home. I do not have a phone, computer, job, and I don't have anywhere else to go for a long period of time. My grandparents can help me for now, but they can barely handle themselves, physically and financially, let alone another person. So, I need to know what numbers to call or websites to visit so I can try to get help. I know there is a way to get a cell phone from the government, but I am unsure of how to get it. I would also need to know if there is a way to be placed in another house to live in as my life at home is unbearable and she is kicking me out when I turn 18 on September 28th. I am going to need help now and then, so any information you can provide is greatly appreciated, thanks.
