How do directors keep cameras out of mirror shots?

Whenever a character is looking in a mirror, the camera is never seen. Sometimes it makes sence, considering the angle the shot is being taken from. But in Black Swan, for example, Nina is dancing in the room with mirrors on the walls and there is no camera in sight. How do film makers do this?

    Well I'm sure they can digitally remove any images they see. But also just really focusing on angles.

    could use two mirrors, one on each side only one of them is a two way mirror. Reflection of a reflection?
