Is is possible to have the USPS forward mail from a PO box to a street address permanently?
Meaning... I have a Post Office box for security reasons (don't want to advertise my home address), but would like to have the mail delivered to my house. In other words - forwarded from the PO Box or redirected from the PO Box to my home address. Is this possible? How would you do this? Is there a form I can fill out online? NO guesses, please. Just actual answers. Thanks.

Letter carrier here, who has had a PO box for 20+ years.
If you want to keep the box open, it's a temporary COA only, and it will expire automatically on the date you provide unless you cancel it sooner. Mail addressed to a PO box can be forwarded temporarily for up to six months.
A permanent address change would be considered the same thing as closing the box, in which case, you can definitely have mail forwarded to you for the 12 month period, and for the six additional months, mail is returned to sender along with the new address.
There is an exception to this, and that's when more than one individual receives mail at the box. An individual can file a permanent COA and the box does not have to be closed.
You can file these forms online at usps.com. There is a $1 verification fee, using a credit card. I prefer the old-fashioned way of handing the filled-out card to a clerk over the window.

No, that is not possible. The maximum length of time for mail to be forwarded is 12 months. After that, and up to 18 Months, the mail is returned top the sender with the new address information attached. A forwarding order is not designed to work the way that you are asking about.
I am not aware of any 2 week time limit on PO Boxes. You may be confused with a CMRA, Private Mailbox company such as a UPS store. But it is still not limited to 2 weeks. The Postal Operations Manual does not distinguish any difference between a City, Rural or PO Box forwarding order.
PS to the retiree, the COA is 3575 but still not bad for being out of the game for 15 years.

Mail addressed to you will not automatically go into your PO box unless you submit a change of address with the post office asking them to forward any mail addressed to you at home to your po box. It is also up to you to see to it that people, banks, credits cards, etc have the PO box # if that's where you want them to send their mail. You need to send them change of address notices. The form from the post office asks who the change of address is for and you will indicate that it is for you only. The form is self-explanatory and I'm betting that if you went to usps.gov you can do it online.

It is possible. The routine way to accomplish this is to fill out a Postal Form 3575, obtained from the counter clerk, and submit it.
With modern technology using the Internet and the USPS's drive to increase sales you could probably submit your application online.
Log on to usps.com/athome or call 1-800-ASK-USPS for a delivery change.
If you prefer to keep your PO box as a buffer between your street address delivery I suggest that you talk with a counter postal clerk for necessary details.
Note: The PS Form 3547 is a change-of-address form used extensively throughout all post offices. The forwarding of your mail will be for 12 months, not two weeks.
2nd note: Obviously, some things have changed since my retirement in 1995. Forwarding service from a post office box is apparently for two weeks (as another responder has reported).
3rd note: Bulkmail is correct; the change of address form is the PS Form 3575. The Form 3547 is used to track address corrections intransit.

I shared my pob with my mom and I moved. The post office said they would not forward unless the box was closed.

No it is one or the other. They will only forward for two weeks but that is considered a Change of Address.
