Alignment for a 1999 mazda 626 4 cylinder? ?
First, how do i know if i need one or not. Second, what kind do i need? And third, any idea on cost?

You can usually tell if you need an alignment by driving on a straight relatively level section of road and seeing if the car pulls to one side or the other if you let go of the steering wheel. This usually also results in it shaking a bit as you force it to go straight.
Make sure your tires are properly inflated, and that they have sufficient tread on them - because they can cause similar problems. If you aren't experiencing any excessive vibration - and the car doesn't pull to one side or the other, you are probably in decent alignment.
I usually only suggest having it checked and aligned when putting new tires on the front (although some vehicles have the ability to align the rear wheels as well). This is mostly just to get the most life out of the new tires though.
It's just another one of those things that they can charge you for (and find other things to charge you for).

It is safe to say, The alingnment need to be check yearly and more often if you drive on bad road. If the is a acident and the front or side of the vehicle is hit. If the there is wear on the inside or the outside of the tire. This car call for a four wheel alignment. Because the back wheel toe can be adjusted. The better the tires roll, the better the gasoline mileage over all. It depend on the labor cost in your area as to the cost. CALL around to shops in your area to get the cost. Don't forget if added work needs to be done it will cost more. Such as shims behind backing plates will cost more plus the shim. Is call setting camber on news car that don't have other ways to adjust the camber. So shim hap to be placed behind the backing plate.

You will know when you need one if your car is pulling to the left or right and you have to hold the steering wheel straight. Also if your tires have uneven wear on them then you need and alignment. Probably cost around 150 dollars. All you need is a 4 wheel alignment.
