I am dating someone who says his ex wife can't watch the little son they have because she has a bad back. So he watches him all weekend.
What kind of bad back can she have at 41 years old that she can't watch her son? Any ideas? He says she is having back surgery in a month?
I really don't know much about it

Sounds like it's a back commitment.
Otherwise known as a bad parent, who doesn't really care about the kids.
Her surgery probably won't go well, I'm sorry for your loss.
