The cost is only the mailing cost which is the cost to mail your package to immigrations registered mail about 20 baht and a envelop with return address with a 5 baht stamp.
1. You need to submit a completed TM 47 - download here http://www.immigration.go.th/nov2004/en/base.php?p...
2. A copy of your picture page and last stamp extension in Thailand.and a copy of arrival/departure card.
3. The original 90 day report notice from your last visit to immigration.
Put all documents along with the return address envelop and mail register mail to immigrations office in your area. Mail it about a week prior to your 90 report date. Figure you will get it back about 3 weeks later if they are anywhere as good as my immigration office.
