Which American Presidents Have Taken Responsibility For A Failure?

One of the regular whines on this forum is that Obama doesn't "take responsibility for his failures" The latest one started me wondering. Of the American Presidents which ones said, while in office, that they had made a mistake with serious consequences?

Update:

No, I'm not asking for "most of them" with nothing to back up that claim. I want actual specific admissions that they have messed up.

    I can think of two. Democratic President Kennedy went on national television and took personal responsibility for the failed invasion in the Bay of Pigs, Cuba. President Kennedy regretted that the invasion failed. Republican President Reagan went on national television and took full responsibility for the Iran/Contra affair. He admitted, " The buck stops here." Both Kennedy and Reagan faced grave consequences for their failures. Both presidents accepted full responsibility, shouldered the blame, and faced the consequences. I admired the HELL out of both presidents.

    James has listed the two that I know: Kennedy and Reagan, unlike Obama who does his best to shirk responsibility for EVERYTHING, including his latest debacle with Syria and throws EVERYONE under the bus: Susan Rice, Hillary Clinton and now John Kerry.

    None that I can think of. Harry Truman did have a sign on his desk that said "The buck stops here," but it was only a sign.

    A number, in fact – the majority, have. Obama more than most, however, always seems to have someone or something to blame for his failures.
