    It's never explained how they met, though probably through something business-related or at a restaurant/bar somewhere. Ranka didn't crossdress before Kotoko's death so he was a normal, if annoying by Kotoko's standards, person. She was probably impressed with his devotion and fell in love similar to how Haruhi fell in love with Tamaki over time.
