Pat, the background is still green and prints as greyscale on my laser printer. This is what I was trying to avoid. Those images are all over the net and that is what you get when you do a search, as I have done several times. I was hoping that someone would UNDERSTAND what I meant when I said "I wanted a downloadable form WITHOUT the green background". I know they exist because I have a filled out one on my desk right now. I don't know where it came from and I can't ask.
