do they decide ahead of time how many of them will win, and the randomness then comes from distributing them randomly, or is the outcome on each one independently determined so that there is no set number of winning scratch cards?

Yes, there are pre determined number of winners of each size. They are distributed randomly during printing.

The randomness comes from dispensing the cards. If a lottery issue a new scratch card game with $10,000,000 in Revenue and $5,000,000 in prizes then the lottery knows it will make a $5M profit when every ticket is sold. The profit will rise when people win and don't claim their winnings.
