Would you ever ignore a friend that wants to hang out with you?

Situation,

Person wants to hang out with a friend of his, and friend says yes, but inexplicably blows them off without notifying the person. Although they remain in contact, the friend never replies and ignores any request to meet up. What does it mean when they do that to you or you do that to them?

Does it matter if it's the opposite gender like girl does it to a guy or a guy to a girl?

If you didn't want to hang out with that friend, would you at least let them know? Like "I just hate you" or "you're too creepy."

If it more than one person that does that you or you and them do it to him/her? What does that mean?

Update:

Well, if you knew a person since high school and years have passed and remained in contact, what would be hard for them simply saying "no" or "I can't because" (for whatever reason)

    I loathe people who cancel without notifying me whether or not they can meet up. I would say its rude. Yes it's strange that a friend would not like to hang out with you. If they are busy at work, they are at a family get together, they are injured, or are busy doing something I'll make that an exception
