Chances or condom failing?

Me and my gf had sex, and we used a condom like we always do. The condom was on the right way and fit well. After I came I pulled out within a few seconds and made sure it didn't slip off. But once I pulled out I noticed the tip was kinda wet and slippery on the outside and it had the consistency of semen but was more clear, and my gf said it smelled like semen a bit. The condom seemed to catch everything on the inside. During sex she was kind of dry down there, and this compounded my worry. This scared both of us, so I went to the sink to fill check for leaks, everything was fine, the condom was completely in tact. Is there a chance she could be pregnant? Was that fluid on the tip my semen or from her. She was close to having an orgasm if that makes any difference.

The fluid was from her, probably fluid as she was getting ready to properly climax. although it has been none that a condom can have tiny microscopic holes in them that only sperm can get through and they can't be seen with the human eye but if you did the water test I'd say it's unlikely that there was any holes..

Women secrete fluids too. I wouldn't be worried but if you are a $49 trip to the pharmacy (Plan B) is cheaper than the alternative.
