Employee Requesting FMLA?

Is there a specific form that is required for an employee to request FMLA? Or do Employers create their own? Is a letter sufficent? New York State if it makes a difference. Thanks!

    Ask your company's human resource department about the documentation required. The Depart of Labor's website has the standard forms used by most employers. You can print them out, take them to your physician, and submit them to your employer.

    Go to your company's h/r office for the form and policy. Many doctors have posted they charge a fee, like around $30, for filling one out as well. Which is common.

    There is no standard form. If the employer uses a form they would create it. If a letter would be sufficient or not would depend on the employer.

    Every place I have worked the company has had specific paperwork they require. Its not a standard form everyone uses but essentially it all says the same thing. Listing dates missed from work, the medical condition and how it would cause you to miss work. It has to be signed and verified by the doc you've been seeing, etc. Ask you manager or HR department, they will be able to get you the proper paperwork they use

    Human resources supplies the form to the employee. They fill it out and take it to their health provider for confirmation, then return it to the employer.

    The FMLA website has necessary forms.

    There is no universal or standard form. Typically the HR department will provide the paperwork needed though. The company (especially if its a larger company) will likely have forms that they use.
