Your cars owner manual will have a section on scheduled maintenance and tell you at what mileage it is recommended to put in a new filter. If you have a K&N aftermarket filter it may be a lifetime filter that just needs to be cleaned and oiled.
You can check the internet for your make model and year and see if there are any owner manuals if you no longer have one. I change mine every 20,000 miles or so, and clean it when i change the oil. Only cost about 20 bucks or so to change it.

Look in the owner's manual for the manufacturer's recommended service interval.
But on most cars, both the engine air filter and cabin air filter are relatively easy to access and inspect. I check mine about twice a year ( and shake the excess debris out of them...that seems to extend their life span ) and replace when necessary.

Check the maintenance schedule in the Owner's Manual - it will tell you when it is recommended to be changed.

That depends on how much you drive it and where. Change it at least annually, more often in dusty conditions.
Take it out and hold it up to the light. If you can't see light through it, replace it.
