(When translating, should I include "hand" since the kanji is there, or should it be implied?)
首を横に振る-Neck horizontally shake...SO: Shake head?? As in to reject the staff? How do I translate this properly?

杖を手に取る-to take up a stick/cane
You don't have to include "hand." It just means to use it for some purpose including walking.
"to reach the cane"
首を横に振る- to shake head to show rejection
"to shake one's head and show disagreement"
