I want to pose as an anti-catholic and say it is a transparent attempt by the Catholic Church to cash in on a famous singers stage name, but I cant bring myself to do it.
It is Italian for, "My Lady". A representation of her with an infant is called Madonna and Child. There were no popular representations of Mary pregnant until our Lady of Guadalupe who is wearing a native piece of apparel worn by pregnant women.

The word is "madonna" and it is medieval Italian for "my lady". Why is it applied to Mary? Well the short answer is, the pagan Roman and Greek cultures were wont to worship mother goddesses, and so when they became "Christians" they generally substituted Mary for Venus, Aphrodite, etc. along with the titles.
