Put in what any application asks for. Name, contact info, previous jobs and phone #'s to them.

Name, address, phone #, education (where you graduated from & when), job history from newest to oldest, company name/address phone, list the dates worked, your tilte, your duties & supervisors name & number.

Depends on what type of job.
Start with the basics. Name, address, phone number, age, current and past employment, experience relative to job and hour and days you're available to work.

Dangerous, this is. They want a sample of your prose, and hand writing for annalists.
Somebody at that company thinks they are knowing psychology...And, are trying to use it as if it was a valid science in human nature.
Get a normal job application.
Write your letter answering in your words all the questions that appear on a normal application.
My opinion is give it a Pass. Find another company.
