If they send you a Friend Request thats how you know they want to be your Facebook Friend or if you send them a Friend Request and they Accept it thats another way to know if they want to be your Friend

you call them on the phone and ask them - other than that, there is no way to be sure - if you dont know them, then adding them as a friend is a violation of the terms of service...

Facebook shows you separate names to people who are linked to the people that YOU are already linked to as friends.
