There are rent places all around Khao Lak and Phuket to rent a boat. You can also google it and get reviews. Just check with a tour operator. But they don't come cheap. You will need to rent the boat and operator. Also, keep in mind if you don't know it already that The Similan Islands are open from mid-October to mid-May. Dates can vary slightly, but for the last two years, the National Park has opened on the 15th October and closed on 15th May.

Both islands are well served by tourist boats out of Thailand. Both have plenty of trips from Patong in Phuket.
