What causes a missed period for 3 months ?

3 months missed period then all of a sudden spotting. Bleeding only when i wipe.

    Potential causes:

    1) Pregnancy

    2) Hormone disorder

    3) Prolonged illness

    4) Malnutrition

    5) Being on hormonal birth control
