Can anyone think of a sample answer for personal effectiveness whether it is a life experience, work, college etc.?

It has to be based on your own specific life experience and cannot be fictional; unless you are going to become a fiction writer. Think of a major conflict that you may have dealt with and how you resolved it; what was it that you did to solve it. It could be a problem with a friend, co-worker, boss,  parent, neighbor, teacher; whatever; including dealing with a major illness; your own or a loved one; loss of a loved one. Even COVID; Identify it first and then list steps that helped you reach a resolution.
