Here's a possibility off the beaten path: If you live in fire ant territory, she could have been laying in the sand and been bitten.
This happened to me when I was a kid in Georgia. I thought I was just in some rocks or something in the sand, but it turned out that I was being bitten by fire ants. They caused small white pustules all over my elbows. The link below will show pics of fire ant bites.
Grandma used some ointment, but I don't recall 30 years later what it was.

My ex-girlfriend used to get something like that, and it turned out to be a fungus. She got some fungucidal creme and it took care of it (she used the over-the-counter kind for athlete's foot). Wash the elbow first, then put on the creme, and be sure to keep using it for at least two weeks, or else it may come back stronger and more resistant to the creme. Good luck. Maybe she's simply allergic to the soap used to wash her clothes. Since underwear is worn in a spot that always rubbed while sitting, and easily sweats, this would cause more irritation. No need to be too worried at this point, just get it checked out with the doctor.

Poison ivy, poison oak, allergy to laundry detergent, could be about a million things. Put some calamine/caladryl lotion on it--- this will take care of it if it is an allergy-type rash/bump. If calamine does not help heal it, take her to a doctor.

I don't know what it is, but I get this sometimes during dry winters. Just apply a heavy-duty lotion or Vasoline on them for a few days, and they'll go away.

Did you put anything made of fiberglass fabric like curtains in the wash recently?

My son has it too. My doc said it was psoriasis. Gave a script for steroid cream and it went away. I would go to doc!
