How to wire a 5V DC relay?

I want to make a toggle or rocker switch to act as a momentary push button switch for keyboard control.

The relay I'm going to use has 5 terminals.

The top 2 terminals I believe are to be connected to a capacitor, switch terminals and DC power supply. Now, I am not sure which terminals out of the 3 left to connect as output i.e. to the control board.

Update:

I hack a USB keybaord to emulate with switches. Since keyboard would take momentary inputs, a toggle switch would repeat input. This circuit is supposed to make the input from a toggle switch to a keyboard as momentary. I don't know how which of the 2 terminals of the 3 from the relay should be connected to the keyboard chip (only 2 inputs).

    Hmmm, it's not clear what you are trying to do. What function do you want the relay to have? Isolate two power supplies? Latch the output? If latch, how do you want to un-latch (turn off) the device. What's the significance of the control board?
