How does an item get proven to be the real thing to earn a certificate of authenticity?

It depends on what kind of item it is. Usually an expert in the category of the item can look at it and decide the authenticity and the value.

It doesn't. A certificate of authenticity is literally just a piece of paper. It's only as good as the company that provides it. The hologram, the embossed paper, the official looking signatures all don't mean didily squat. Most collectibles sold over the internet or through TV infomercials are worthless, even with a certificate.
