I was going to suggest Word as well and see if you can alter one of their templates to your dimensions. I found a really nice one that I like and will probably use from searching on the microsoft.com template section.

If you're using microsoft word, you can set-up a table that will let you divide the page in half. Go to file, page set-up and select either portrait or landscape. Then fix your page margins. After that, go to table, insert table. Select three columns, 1 row. Make your second colum pretty small to use as the spacer between the two rows. Type your stuff in the first column, get all your spacing right (vertically and horizontally). Then copy and paste to the third column. Print on regular paper, place it under your blank program, hold it up to the light and make sure it is going to look the way you want it to. (Be sure to save your file before printing). Make any adjustments then and you should be ready to roll!

you should create your own using microsoft word... you can just adjust the paper size under "print options" and then use two fonts (one of the titles and one for the response, ie... bridesmaids and their names) you can even find some classy clipart or save images you find on the web
