They'd be stupid not to. After that point, the erection would get painful, and it would start to damage the blood vessels in the penis. When you have vessel damage, you can wind up irreversibly impotent (the kind of impotence Viagra cannot fix).
Don't be embarrassed. Doctors -- even small town docs -- have seen a LOT. And a case of priapus (prolonged erection) would not even be remarkable.
Don't be the guy who walks into the ER with a goat's horn up his butt, though. That'd be too funny.
