Is it okay to begin a sentence with, "as well"?
Example:
"As well, you are required to submit your proposal electronically."

It is not grammatically correct to start a sentence as you have stated. A better way would be -
You are required to submit your proposal electronically as well. (Meaning 'also', 'in addition to')

The sentence you have formed is not, infact gramatically correct.
A more appropriate way to phrase this sentence would be: "You are also required to submit your proposal electronically".
OR
"In addition, you are required to submit your proposal electronically"

I can think of a couple of occasions when it would be gramatically correct, and not at all awkward, to begin a sentence with the words "As well".
As well as I know, all went according to plan.
As well as you dance, you should consider a career in dance.
As well as we could ascertain, everyone seemed to have survived the finals.
As well as losing their home, the Smith family also lost several well-loved pets in the fire.
As well as I have felt all today, I fear I will be sick tomorrow.
As well as I am able, I believe I have made my point.

no... sounds bad.
normally it is "as well as".
It is not proper to begin the sentence with "As well".
"All proposal submissions must be transmitted electronically".

Starting a sentence with 'As well' is primarily a Canadian English construction and as such it is perfectly acceptable in both spoken and written English.

Is this sentence correct?
As well as, a dietician review for weight loss, he has been increasing his appetite and has gained weight of 2kg.

I don't think so, unless you start it maybe like this "As well as your identification document, please submit your ....................
