Left-handed:
Johan Santana

Cole Hamels
Jon Lester
Scot Kazmir
Brian Fuentes
Or
Right-Handed:
Tim Lincecum
Jake Peavy
Brandon Webb
Carlos Zambrano
Rich Harden
Brad Lidge

Sabathia cancels out Lincecum
Hamels cancels out Webb
Johan cancels out Peavy
Lester cancels out Harden
Kazmir cancels out Zambrano
Bridge>Fuentes
I would take either in a second, but I'd have to go with the righties.

Replace Scot Kazmir and Brian Fuentes with Cliff Lee and Billy Wagner (you did say completely healthy) and I'll take the Left Handed squad.
edit: If you don't get to replace people then I'll go with the Righties. By the way Cliff Lee has been 14-8, 18-5, and 22-3. That's 3 good years. Lincecum has only been good for 1 year and Kazmir's best year is 12-8.

The righthanders, because of Lincecum and Harden. Harden can pitch as good as Lincecum when he is healthy, so assuming Harden is always healthy, he really helps the rotation.
I would probably replace Zambrano with AJ Burnett or even someone like Volquez or Billingsley. Zambrano had a no-hitter, but the rest of his season was pretty average for him.
One more thing, there are more right handed hitters in the league than lefties giving the RH pitching rotation an advantage.

Tim Lincecum
Jake Peavy
Brandon Webb
Carlos Zambrano
Rich Harden
Brad Lidge
Although I would put Mariano Rivera as my closer, but Lidge was a good pick.

righties. Rich Harden, when healthy, is quite possibly the most dominant pitcher in the league (he's rarely healthy though). Zambrano is a horse and keeps getting better as the game goes on. Peavy... I don't think he has an equal. He's amazing.
As good as the lefty squad is, the right handers have an advantage.

The lefties are better because they have the two best pitchers in baseball (Santana and Sabathia). Hamels is one of the only pitchers who can survive the Phillies' park and it's impossible to get more than a linedrive off him. Kazmir is a five inning pitcher, but so is Harden so it balances out. Lidge is better than Fuentes, but the rotation is much more important.

Tough choice here, but when healthy Rich Harden has the nastiest stuff in the game. My edge goes to the righties!
p.s. I believe Lee had a good year in 05 or 06

right handed.... Though I question your choices for RH pitchers on that list... No Josh Beckett??? Mariano Rivera is better than Lidge its no question...
