Do get him in to see a Cardiologist ASAP. Take him to heart(cardiac) clinic. Just it do! Sleeping on his side is better. Is he watching in salt intake? If not, he needs to cut back on salty foods and reduce his fluid intake, this will help reduce some the fluids building up. Since he know he has enlarged heart, he know he needs to see a Dr.. Is he taking any heart medications? God bless

Heartworms can cause an enlarged heart as well as Congestive Heart Failure (which would explain the fluid in the lungs).. have you had your dog on HW preventitive or has he been tested??

That is a very urgent matter. He needs to see a doctor ASAP. Anything that directly affects the heart should not be taken lightly. Tell him if he cares about HIMSELF and YOU then he will go get it checked out. It could be a simple fix for all he knows.
