The philosophical answer is we are separated by what we or either one of us places between each of us.
The anthropological answer is all humans cannot develop logical thinking until our individual mind builds a barrier between what we think of as our Self and everything else in the Universe. This boundary is often called the Ego boundary. It separates You from Me as well as Self from Universe.
Some adults (and only some) begin to question this boundary and it's consequences. These people tend to find that Reality is Unity and they are more comfortable eliminating personal judgments that separate us.
What brings You and Me together are those thoughts, feelings and actions which tend to cross these personality boundaries or lessen them. Love, compassion, empathy, honest communication of both personal thoughts and feelings and true understanding will all promote harmony.

Playing after the answer from Just be, lacking of unity is the biggest problem I have seen, for an answer to this.
If you have ever heard the song, from my favorite band from my state of Colorado, Rise by The Flobots; you may have thought about the problems that a lack of unity creates. Truly, we all do "rise together" and likewise, fall together. Or if you've ever listened to Schism, by Tool. Both of these songs speak about the problems and doomed possibilities of life without stronger attempts to work together...
Separation is an attempt of control, sometimes. Or it is a cause of laziness, or some other form of negative destruction...
The words "You" and "Me", can, and sometimes have, become means to create seperation amongst us. Not always intentionally, or for a negative purpose, but for different reasons for everyone. The mind creates seperation to deal with things. Sometimes it works, but a lot of times it isn't the best way to deal with problems...
Truly, the acts which people consider to be "of Positive intention" are what brings us all together. Unfortunately, the acts which have "no intention", or a "negative intention", work to ruin past positive creations. out of the 3 possible intentions behind everything, 2 destroy, and 1 creates.
Each intention has a direction. Sometimes that direction is not achieved, but usually it is. The idea/concept of "no intention" is usually a lack of will to do anything. So no change is instilled, and "time's intention" is always working. Time can mend certain problems, or make others worse.
Just as has been said before, seperation is an illusion. The idea, that we have something to distinguish between one another, was a created perception. A cloud of fog that we, sometimes, notice as being described as very thick. Bringing us together, is when we try to clear away all of the fog, and perceive without the illusion of seperation.
Sometimes it brings us together, but then we move on to other assumptions about seperation. i.e. being around someone, because they have a positive effect on you, generally speaking. That is a form of seperation. But it is perceived, as a positive seperation. So it is "acceptable"...
So once again, we come full circle back to the concept of Perception...

Nothing separates us; we are one.
Just because one believes we are separate doesn't make it so.
Identification with the ideas of "you" and"me" as separate beings makes one believe we are separate. It's just not true, so there is nothing needed to bring us together.

Apparently, language separates us, as does personal history, genetics, and locality. Now, what brings us together? Shared values, respect for diversity, compassion, the I/Thou relationship that occurs between creation and identity, and, perhaps most importantly, a willingness to recognize and share the One Love that binds us all to each other and to the sublime.

That very labels that were chosen to represent "you" and "me" lead to the mistaken and errant belief that one is separate from all else that has arisen in the cause and effect manner of interdependence.
"You", "me", "I", do not exist and such terms only represent the "ego" and, thereby, a false idenity.
Each is "one" and a part of the oneness of all that exist.
Understanding such and living in harmony with that which is the essence of all that is awakens every one to the reality of being.

as long as we think in terms of 'you' 'me' (or 'black/white' jew/palestinian' 'democrat/republican'.........) we will be seperate to be together we must look and ask about whats best for 'us'
when we are no longer leemings and society as individuals discover the joy in humanity we will also be 'us' as well as'you' and 'me'
but together we will be a force that will change the world
