We speak the same language, it's just the accents and a few odd words pronounced/spelt differently. I'm English and i think most English can work out the difference. However it could be confusing. At school, I used to have difficulty spelling a few words like 'colour' and 'metre' because i know i've seen it spelt differently. It was bloody annoying although i think we just all learn and get used to it. We had an american teacher at school and used to take the piss out of him for his accent and when he said words differently like calling football soccer. It was funny but we understand clearly what he means. Also with the difference in words, as you get older and used to it we learn the american words. Things like 'trunk', 'sidewalk' and 'cell phone', i never knew what it was until learning from experience and watching american programmes like the Simpsons and Family Guy. Or to answer your question simply... No we don't have trouble with their language or need subtitles but it can be annoying at times.
