Need a blank copy of CA Highway Patrol Accident - Ivestagation Report...?

Form name/#: CHP 216 (Rev. 1-99) OPI 004 The copy of the completed report given at the CHP dept. has PAGE 1 - _ (?), auto filled in but the copy i have the number indocating hpw many pages is suppose to be there wad white out and hand written in. I am trying to download the pdf or other doc type of a blank copy to find out how many pages the document should be as i only was given two pages but believe there should be/is more pages than just 2. I check online and on the official chp website by entering the form name but could not find it.... any suggestions or other websites i could try for this information? Any help or suggestions on how to tell how many pages this report should contain woulf be very helpful.. thank so much.
