Saikano, The Last Love Song on This Little Planet.
The subtitle kind of gives it away. At least the lovers survive and they will stay in love. This is actually a live action show, but there is also a manga.
This one hasn't been made into an anime yet, but I guarantee you will like it:
Bitter Virgin. This is one of the most heart-aching stories I have ever read, partly because the mangaka says it is based on her real-life experiences. You will feel so sorry for the main character. A selfish, insensitive guy accidentally hears a deep dark secret about a girl he's not interested in and starts watching her to find out if it's true. He learns to care for her and grows from being a stuck up stupid kid into a kind, caring young man. There are so many cliffhangers! But there is a happy ending. I have read this over and over and don't get tired ot it, it is so beautiful. Make sure you have tissues handy.

Air Kanon and Clannad are MUST BUYS! They are the saddest anime out there! Also try Clannad After Story!

I highly recommend "The graveyard of Fireflies" im not sure where you can get but I seen it On Demand. The anime is the only anime that made me cry.

"clannad" is a good choice, so as its sequel, "clannad: after story". it's worth buying the animation.
you should watch the sequel straight after the first. i can guarantee you getting yourself a pair of red eyes; if not, then it's alot of *sniff* *sniff*
