When you register a pet (cat or dog) do you have to give them your last name?

I can answer you about a cat. I will discuss how to register with CFA and TICA, the two largest cat registries.
If your kitten/cat comes with a "blue slip" it will have the name of the cattery that the kitten came from first. Example my cattery is "Vasilis", so the blue slip you receive will have the name, "Vasilis" already typed on the blue slip. You have a total of 35 spaces to pick out the cats name. Example: "Vasilis Natasha", Natasha being the name you gave your kitten. There will not be a last name on the kitten. Like with my breeding cats, the breeders name where I bought my cats name is first and then the name I named my cats and then the name of my cattery. Example, "Mountain Fork Sasha of Vasilis". Mountain Fork is the breeder I bought my cat from, Sasha is the name I gave her and 'of Vasilis' is my cattery's name. Once you send in your blue slip, filled out with your name and address and payment, you will receive a "green" registration paper back from CFA with the name of the cat as you registered it first, then the Dam and Sire of the kitten next and then the name of the breeder in the bottom right hand corner and your name/address, the owner of the cat, in the bottom left hand corner.
With TICA the female cat (Queen/Dam) "must" be registered with TICA in order to register her kittens with TICA . Now if you are "importing" a cat to TICA you need a registration document from another certified registery, plus a 3 generation pedigree (including names, colours and registration numbers) signed by the breeder of the cat. When this is done the cat that is imported will have the exact same name that is on their CFA papers, ex: "Mountain Fork Sasha of Vasilis". Mountain Fork (the cattery she came from) does "not" have to be registered with TICA. My cattery is registered with TICA thus the reason I have at the end of her name, "of Vasilis". With TICA you can change the name of the cat within 45 days of the original issued registration papers for a fee of $50.00 as long as the cat has not had kittens, sired kittens or has been shown and awarded any ribbons.
I hope this helps and I have not confused you. :)

Tether all 3 dogs in the room with you and ask for a down stay or sit. While all 3 are looking at you start with the 7yr ches/lab and call her name then walk over and give treat/toy with lots of pets/praise. Then do the other two one at a time. Do this about 2 to 3 times a day. If they do fine without being tethered to something than go with that. Try to make your older dog 7 yr first for attention. Also give a extra finishing one to the older one. When doing some obedience such as Sit, start with the older one and praise, then go to the next two.

No. If its a legit registry like CFA, ACFA, or TICA, the cat is registered with the breeder's cattery name first (if they are registered in that association), then the cat's name.
Your name is as owner - the cat never gets your last name as part of the registration.
Some examples:
Cat is registered in CFA (breeder has a cattery name)
Moonbeam's (cattery name) Star Dust (the cat's name you picked)
John Doe (owner)
Cat is registered in TICA but breeder is not; only registered in CFA
Star Dust (the name you picked).
The Moonbeam would not be part of the registration name because the breeder (above) was only registered in CFA and not TICA.

Usually the vets do that. I remember taking my cat to the vet a few months ago. The vet called my cat by my last name. Trixie such and such. I thought is was funny.
