Abortion and Medical Ethics?

When a child (girl 12 or under) becomes pregnant (however it happened) is there any 'rules' that doctors use to determine that an abortion would medically necessary?

Does the child have a choice or is she ineligible to give an opinion by her ignorance and age?

The doctors can never decide to give someone an abortion even if it was deemed "medically necessary" the legal guardian would need to give approval. However if the parents approved it and the 12 year old didn't want it then she could contact a lawyer or someone along those lines to have her rights represented.
